(a) Jobsite, 1892 Central Ave. Albany, NY- On or about 05/02/2012- Employee was not equipped with a personal fall arrest system when working and standing on the top rail of a genie scissor lift. Employee was exposed to a fall hazard greater than 10' to ground level. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a personqualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to controlor minimize those hazards: (a) Jobsite, 1892 Central Ave. Albany, NY- On or about 05/02/2012- Employee was not equipped with a personal fall arrest system when working and standing on the top rail of a genie scissor lift. Employee was exposed to a fall hazard greater than 10' to ground level and was not trained in the recognition of the hazard. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM
