Oven energy sources were electrical and air. Only two padlocks were used to isolate Oven's energy sources instead of using four padlocks. ABATEMENT CERTIFICATION REQUIRED

Therefore, when setting up this machine, employees could place one hand on the jog button and the other hand on the point of operation exposing them to amputations. ABATEMENT DOCUMENTATION REQUIRED
