29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: The employer does not ensure that all respirators are stored so that they cannot become contaminated. This violation was observed on or about May 1, 2012, in the sand blasting area of the facility when a supply air respirator was found stored on the floor on top of used blasting material.

29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed: The employer does effectively close all openings through which conductors enter cutout boxes, cabinets, and fittings. This violation was observed on or about May 1, 2012, in the sand blasting area of the facility when an opening in a junction box that an electrical cord was entering was not effectively closed.
