29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch. The employer does not ensure that work rests on bench grinders are adjusted properly with respect to the abrasive wheel. This violation was observed on or about May 1, 2012, in the welding area, where employees were exposed to struck-by hazards when operating a bench grinder with an improperly adjusted work rest which was one inch from the abrasive wheel.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch. The employer does not ensure that tongue guards on bench grinders are mounted properly. This violation was observed on or about May 1, 2012, in the welding area, where employees were exposed to struck-by hazards when operating a bench grinder with the tongue guards mounted improperly, creating a gaps of one and one fourth inches.
