This violation was observed on or about April 30, 2012, when employees throughout the facility were exposed to struck-by hazards while blowing off equipment with compressed air that was set to a pressure of nearly 60 p.s.i.

29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel:  The employer does not ensure that all angle grinders used by employees are equipped with a guard.  This violation was observed on or about April 30, 2012, when employees working throughout the facility were exposed to struck-by hazards while using angle grinders that were not guarded.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  The employer has not implemented a hearing conservation program for employees that are exposed to noise levels at an eight hour time weighted average greater than 85 dba.  This violation occurred on or about April 30, 2012, when employees working throughout the facility were exposed to noise levels at eight hour time weighted averages ranging from 89.9 to 106.4 dba.

29 CFR 1910.134(h)(1)(i): Respirators issued for the exclusive use of an employee were not cleaned and disinfected as often as necessary to be maintained in a sanitary condition:  The employer does not ensure that all respirators used by employees are cleaned and disinfected as often as necessary to be maintained in a sanitary condition.  This violation was observed on or about April 30, 2012, in thermal spray booth #2 when a 3M tight fitting half face respirator an employee was using was found in an unsanitary condition.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:  The employer does not ensure that all respirators are stored so that they cannot become contaminated.   a. This violation was observed on or about April 30, 2012, throughout the facility when employees stored 3M half face tight fitting respirators in an area contaminated with metal dust.  b. This violation was observed on or about April 30, 2012, throughout the facility when employees stored supply air respirators on the floor.
