29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shallbe avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. (a). Worksite located at 1218 Village Drive, St. Joseph, Missouri- Employees were exposed to a shock or electrocution hazard in that an extension crord was run through an aluminum window frame in which the frame posed a pinch point for the extension cord.

29 CFR 1926.453(a)(2): Aerial lifts may be "field modified" for uses other than those intended by the manufacturer provided the modification has been certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI A92.2-1969 and this section and to be at least as safe as the equipment was before modification. (a). Worksite located at 1218 Village Drive, St. Joseph, Missouri- The employer had field modified an aerial lift without the writen certifed approval by the manufacturer to perform this modification.
