29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees:      On or about February 14, 2012, at the 7800 block of Gateway East in El Paso, TX, employer did not deactivate the natural gas pipe while being replaced. The excavation was not reinforced with a shoring system to prevent cave-in. The excavation collapsed and a gas line exploded. Employees were exposed to a gas line exposion, natural gas overexposure and potential fire hazards.
