29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identified the workplace evaluated; the person that certified the evaluation had been performed; the date(s) of the hazard assessment; and identified the document as a certification of hazard assessment.    a) On or about May 1, 2012, at the above located workplace, employees were exposed to hazards that required the uses of personal protective equipment. The employer had not verified through a written certification that the required workplace hazard assessment had been completed.

29 CFR 1910.1030(c)(1)(i):  The employer having employee(s) occupational exposure did not established a written Exposure Control Plan designed to eliminate or minimize employee exposure:    a) On or about May 1, 2012, at the above located workplace, one of an employee's collateral duties included rendering first-aid which may result in exposed to blood. The employer had not established a written Exposure Control Plan designed to eliminate or minimize employee's exposure.

29 CFR 1910.1200(e)(1): Employers did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met:    a) On or about May 1, 2012, at the above located workplace, employees were exposed to hazards associated with chemicals such as, but not limited to, machine oil, paints, strength cleaners and other hazardous chemicals. The employer did not maintain a written hazardous communication program.
