29 CFR 1910.95(c): 29 CFR 1910.95(c)(1): A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employees noise exposures equaled or exceeded an 8-hour Time Weighted Average (TWA) sound level of 85 dBA: On or about May 28, 2013, at the Parts Washing Machine Area, employees were exposed to noise in excess of an 8-hour Time Weighted Average of 85 dBA and a continuing and effective hearing conservation program had not been implemented: a. An employee washing parts was exposed to continuous noise at an 8-hour time-weighted average sound level of approximately 91.6 during a 203 minute sampling period on May 28, 2013. Exposure calculations included a zero increment for the 277 minute period not sampled.

Parts washing machine area - On or about May 28, 2013, employees used compressed air measuring approximately 50 p.s.i to clean parts and their hands and body. Employees were thereby exposed to the hazard of air embolism. b. Parts washing machine area - On or about May 28, 2013, employees used compressed air measuring approximately 74 p.s.i. to clean parts. Employees were thereby exposed to the hazard of air embolism.

Chip Spinning & Parts Washing Machine Area - On or about May 28, 2013, an orange electrical extension cord used to power a Dayton crane was missing a segment of outer electrical insulation exposing the interior individual wires.
