29 CFR 1910.303(g)(1)(vii)(B): Outdoor electric equipment was not installed in suitable enclosures and was not protected from accidental contact by unauthorized personnel, or by vehicular traffic, or by accidental spillage or leakage from piping systems. (a) At the sand stacker, the Allen Bradley (AB) outdoor electrical junction cabinets and transformers were not protected from vehicular traffic on the terminal dock area.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: (a) At the Bettendorf terminal the AB electrical cabinet supplying 120/240 volt service to the sand stacker had an unused opening (fitting) that was wrapped in electrical tape that was not effectively closed and the cabinet was not protected from the weather nor insects that could access the internal conductors of the cabinet. This condition allowed weather and insects to enter the cabinet and created an electrical hazard to employees.

29 CFR 1917.112(a)(1): Vehicle curbs, bull rails, or other effective barriers at least 6 inches in height were not provided at the waterside edges. (a) At the Bettendorf terminal's waterside edge, openings in the barrier approximately 12-15 feet wide and 6 feet wide were not guarded to prevent vehicles from driving overboard during operation. A John Deere Excavator 350D (1FF350DXTA0807068) was unloaded from a flatbed truck near the unprotected edge. (b) At the Bettendorf terminal's waterside edge, openings in the barrier approximately 12-15 feet wide and 6 feet wide were not guarded to prevent vehicles from driving overboard during operation. A John Deere loader (DW644GB541935) was operated near the unguarded edge clean up any sand spilled during processing and smooth out the travel area.
