29 CFR 1910.132(a):     Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    a. On or about May 4, 2012, in the wheel rim stripping area, employees required to handle rims treated with ethanolamine were not provided adequate personal protective equipment including, but not limited to, chemical jackets.  In addition, chemical pants worn by the employees had holes and/or cracks that would permit chemical exposure.

29 CFR 1910.132(f)(1):     The employer shall provide training to each employee who is required by this section to use PPE. Each such employee shall be trained to know at least the following:    a. On or about May 4, 2012, in the dip tank area, the employer did not provide adequate training to employees on the limitations of the personal protective equipment (PPE) and the proper care, maintenance, useful life and disposal of the PPE when working with chemicals such as ethanloamine.

On or about May 4, 2012, in the dip tank area, the employer did not ensure the use of safety goggles when employees were required to work by the dip tank that contained ethanolamine.

29 CFR 1910.134(c)(1):     In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer shall include in the program the following provisions of this section, as applicable:    a. On or about May 4, 2012, in the dip tank area, employees were required to wear a half mask respirator, but the employer did not implement portions of the company respirator program including, but not limited to, medical evaluation, training and fit testing to ensure compliance with the provisions of this section.

Employers shall base the selection of the appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the task(s) to be performed, conditions present, duration of use, and the hazards and potential hazards identified.  [59 FR 16362, April 6, 1994]    a. On or about May 4, 2012, in the dip tank area,  employees were required to work with ethanolamine. The glove selected by the employer was not appropriate to protect the employees arms and hands from chemical contact.

29 CFR 1910.1200(h)(1):     Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.    a.
