On or about May 3, 2012, at the job site, employees were installing roof decks on a 3-story building approximately 30 feet above a lower level.  The employees were not wearing personal fall arrest system and no other type of fall protection system provided.  Employees were exposed to fall hazards.     THOMAS HINES CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314932419, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 21, 2011 WITH A FINAL ABATEMENT DATE OF JUNE 25, 2011, WITH RESPECT TO A WORKPLACE HAZARD AT 401 WEST COVELL ROAD, EDMOND, OK 73003.
