29 CFR 1926.453(b)(2)(v):  A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.    a.  On or about May 4, 2012 - at the above addressed jobsite, an employee working in a TB42 Snorkelift telescopic boom aerial lift did not have a lanyard secured to the basket, thereby exposing the employee to the hazards associated with a fall of approximately fourteen (14) feet.    b.  On or about May 4, 2012 - at the above addressed jobsite, an employee working in a TB42 Snorkelift telescopic boom aerial lift was not wearing a body belt or full body harness, thereby exposing the employee to the hazards associated with a fall of approximately nineteen (19) feet.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. On or about May 4, 2012 - at the above addressed jobsite, an employee performing carpentry operations from the roof was exposed to falls, of approximately nineteen (19) feet, created by the presence of unprotected sides, thereby exposing the employee to fall hazards.
