29 CFR 1910.134(c)(1):     A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  (a) (LOCATION) (IDENTIFY SPECIFIC OPERATION/CONDITION) (DESCRIBE HAZARD)      a) At the machine shop - The employer did not provide a written respiratory protection program to employees wearing the 3M - 5203 Medium - Organic vapor/acid gas assembly dual cartridge respirators and 3M NIOSH dust masks voluntarily when welding and working with stainless steel, steel, and aluminum at the working area, exposing employees to the respiratory system irritation and permanent respiratory disease.

29 CFR 1910.134(c)(2)(i):     Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:        a) At the machine shop - The employer did not provide information contained in Appendix D of Respiratory Protection program to employees permitted to use 3M - 5203 Medium - Organic vapor/acid gas assembly dual cartridge respirator and 3M NIOSH dust mask respirators voluntarily when working at the shop, exposing employees to the respiratory system irritation and permanent respiratory disease.

29 CFR 1910.253(b)(2)(ii):     Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards.    a) At the machine shop - The employer did not store flammable gas cylinders such as, but not limited to, propane and acetylene cylinders in the well-protected and safe locations employees working at the shop, welding stainless steel metal sheets and operating the forklift, exposing employees to the fire and struck by hazards.

29 CFR 1910.253(b)(4)(i):     Oxygen cylinders shall not be stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near any other substance likely to cause or accelerate fire; or in an acetylene generator compartment.      a) At the machine shop - The employer stored the oxygen cylinders near highly combustible gas cylinders such as, but not limited to, acetylene cylinders next to the exit door at the machine shop area where employees were welding stainless steel , aluminum, and copper and operating forklift, exposing employees to the fire hazards.

29 CFR 1910.1200(f)(5)(ii):     The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the appropriate hazard warnings:      a) At the machine shop - The employer did not maintain each container of chemicals such as, but not limited to, Honing oil and  Trim Sol with the labels and tags for the identity of the chemicals, hazard warnings, and general information regarding the hazards of the chemicals, exposing employees to eye, skin and pulmonary irritations and dermatitis or oil acne.

29 CFR 1910.1200(h)(1):     Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.    a) At the machine shop - The employer did not provide the information and training on hazardous chemicals to employees working with chemicals such as, but not limited to, stainless steel, chrome plate carbon steel, aluminum alloy, and propane , exposing employees to eye, skin and respiratory system irritations and fire hazards.
