OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fire hazard.      A) Warehouse #6, on or about April 27, 2012, fire extinguishers were missing from there indicated location, exposing employees to a fire hazard.    Among other methods, one feasible and acceptable abatement method to correct this hazard is to replace missing fire extinguishers in the warehouse area.    ABATEMENT CERTIFICATION IS NOT REQUIRED.

29 CFR 1917.43(c)(5):     Powered industrial trucks were not maintained in safe working order.    A) Hazards-Operation/Condition-Accident: Employees were exposed to a struck by hazard.  Employees operated a Toyota forklift that did not have an operable back-up alarm. The employees used the forklifts to load material into the back of a tractor trailer.  The operator was observed backing up without looking in the direction of travel.       B) Employees were exposed to a struck by hazard.  Employees operated a Belotti reach stacker to move containers.  The Belotti reach stacker did not have an operable horn or back-up alarm. The operator was observed backing up without looking in the direction of travel.       NOTE: Liberty Terminals was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1917. 43(c)(5) which was contained in OSHA Inspection Number 314095779, Citation Number (1), Item Number (1), Issued on  2/01/2012 and was affirmed as a final order on 2/27/2012 with respect to a workplace located at 112 Marine Terminal Dr., Savannah, GA.    ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.      ABATEMENT CERTIFICATION IS REQUIRED.
