29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided in this paragraph by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels. a) USA Production Parts, Inc. - An employee operating the Small Roll line was exposed to noise levels of 89.6 dBA, exceeding the action level of 85 dBA. An audiometric testing program was not established. The exposure was obtained for a period of 473 minutes, on or about 4/30/12. b) USA Production Parts, Inc. - An employee operating the Raping Strap Press was exposed to noise levels of 88.5 dBA, exceeding the action level of 85 dBA. An audiometric testing program was not established. The exposure was obtained for a period of 473 minutes, on or about 4/30/12. c) USA Production Parts, Inc. - An employee operating the Punch Press LP Georgetown was exposed to noise levels of 87.7 dBA, exceeding the action level of 85 dBA. An audiometric testing program was not established. The exposure was obtained for a period of 473 minutes, on or about 4/30/12. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
