29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm:    (a)      At the VA Central Ia Health Care facility in Des Moines Iowa humidity condenses on the walk in freezer floor  and    freezes creating slip and fall conditions for employees who enter and exit the freezer.  Condensate from the evaporators also drips on the floor in a portion of the freezer area where employees walk.  Since the installaton and use of the freezer employees have fallen and incured bruising  and time off injuries.  A reported fall has occured as recently as May of 2012.  The employer knowing of the hazard has attempted various repairs and engineering controls to eliminate the icing but to date has not accomplished the elimination of icing.      Abatement certification and abatement document is required for this violation.  The documentation should include written verification of abatement, applicable measurement or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).
