29 CFR 1910.1200(e)(2)(i): At a multi-employer workplace, the employer's hazard communication program did not include methods the employer used to provide other employers with on-site access to a material safety data sheet for each hazardous chemical the other employer(s)' employees may be exposed to while working:    (a)  Although the Central Iowa Veterans Health Care System had a written hazard communication program that addressed contractors, the program was not implemented to ensure access to Material Safety Data Sheets for galvanized steel, E7018 welding rods, and SprayOn Zinc-Rich Cold Galvanizing Compound.  US Erectors were welding on these products in Building 3 on the entry stairs project and exposed VA employees to the welding fumes.

29 CFR 1910.1200(g)(8): The employer did not maintain copies of the required material safety data sheets for each hazardous chemical in the workplace:    (a)  The Central Iowa Veterans Health Care did not maintain Material Safety Data Sheets for hazardous materials, such as but not limited to, galvanized steel, E7018 welding rods and SprayOn Zinc-Rich Cold Galvanizing Compound where contractors work processes involved these materials in Building 3 entry stairs project, exposing VA employees to hazardous materials.

29 CFR 1910.1200(e)(2)(ii): The employer's hazard communication program did not include methods the employer will use to inform the other employer(s) of any precautionary measures that need to be taken to protect employees during the workplace's normal operations and in foreseeable emergencies:    (a)   Although the Central Iowa Veterans Health Care System had a written hazard communication program that addressed contractors, the program did not address the precautionary methods to be used when their own employees were exposed to chemicals, including but not limited to welding fumes associated with welding on galvanized steel using E7018 welding rods and SprayOn Zinc Rich Cold Galvanizing Compound.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    (a) Under the multi-employer worksite policy, the Central Iowa Veteran Health Care System, whose own employees were exposed to chemical hazards of US Erectors welding operations did not inform their own employees of the specific health hazards associated with the chemicals used in the welding process.   The hazardous materials included but not limited to E7018 welding rods, ASTM A36 steel beams galvanized, Sherwin Williams SprayOn Zinc-Rich Cold Galvanizing Compound.
