29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) Jobsite, Green & American Sts: Employees were conducting stucco prep operations from a fabricated frame scaffolding approximately 30' in height without adequate means to safely access the work platform levels beyond the third platform. Portable extension ladders onsite only reached to the third work platform level. Employees were observed working from the fifth work platform level, observed on or about 4/18/12. JC CONSTRUCTION WAS PREVIOUSLY CITED FOR THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314232984, CITATION 1, ITEM 1c AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 15, 2011 WITH RESPECT TO THE WORKSITE LOCATED AT 1136 S. 23RD ST, PHILADELPHIA, PA
