29 CFR 1910.212(a)(3)(ii): The point of operation of machines was not guarded: a) Plant - On or about 05/01/12 the 16 Ton. Roper Whitney bending brake model AB1016T, SN 147-12-07 was not guarded. b) Plant - On or about 05/01/12 the 14 Ton. Roper Whitney bending brake model AE1014, SN 483-1-98 were not guarded. c) Plant - The back of the Tennsmith 10 ft. shears was not guarded. In accordance with 29 CFR 1903.19(d), abatement cerification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
