29 CFR 1926.453(b)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket while working from an aerial lift:    (a)  S&J Ford Construction LLC on or about May, 09, 2012, at the address of 141 Innovation Dr. Elyria, OH: The employer failed to ensure employees working from the JLG aerial lift were adequately protected from falls.  Employees were not clipped off to the basket of the stated aerial lift while the basket was elevated. The employees were exposed to an approximately 32 ft fall to the ground below.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a)  S&J Ford Construction LLC on or about May, 09, 2012, at the address of 141 Innovation Dr. Elyria, OH: The employer failed to ensure employees installing roof blocking atop a commercial structure were protected from a fall. The employees working in this condition were exposed to an approximant 32ft fall to the ground below.
