29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest system: On, or about, April 11, 2019, employees working on the roof (4/12 pitch) of Akron Fire House # 2, located at 952 E. Market Street, Akron, OH 44312, were exposed to 18 feet falls. Employees were engaged in roofing activities and the employer did not ensure all employees used fall protection.
