29 CFR 1910.217(c)(3)(iv)(b): Attachments shall be adjusted to prevent the operator from reaching into the point of operation or to withdraw the operator's hands from the point of operation before the dies close. In the production area of the plant, the employer used "pull backs" for employees who operated presses to protect the employees from the point of operation of the press. The "pull backs" were not properly adjusted to prevent the employee's hands from entering the point of operation on the Multitech 2301 coin die press #400. On or about March 20, 2012, an employee assigned to mechanical power press (Multitech 2301coin die press) #400 was injured when his hand was caught in the press during a cycle. The employee's left index and middle finger tips were amputated and his middle knuckle was broken.

29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. a. At the worksite located at 1120 C.I.C. Logan, Ohio, on the day of inspection, pallets were stored in the aisle way creating hazardous blind spots. The shipping area was a regular/permanent passageway for forklifts and pedestrians. Two employees were injured when struck by a forklift in the shipping area.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. At the worksite located at 1120 C.I.C. Drive Logan, Ohio, in the cutting area, there were 3 unguarded rotating turn tables (approximately 4-5 feet diameter) that fed spools of solid steel wire (approximately 4 feet in diameter) into the cutting machines. Employees were exposed to hazards created by the rotation of the turn tables and being struck by the steel wire whipping when the end of the cable was spun off of the spool.
