29 CFR 1926.652(d)(2): Manufactured materials and equipment used for protective systems were not used and maintained in a manner that was consistent with the recommendations of the manufacturer: a) Excavation Area: The Efficiency Production, Inc. hydraulic aluminum shoring, protective system, was not used in accordance with the manufacturers tabulated data, on or about 5/10/2012. The faces of the excavation were not straight and near vertical. The shoring members did not bear on firm soil or solid filler. The soil was not able to stand with unsupported vertical sidewalls long enough for shoring installation. Trenches exceeding 8 ft. in length did not have a minimum of 3 shores spaced according to the tables.
