29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).      Less Than 300 V - minimum distance required - 3 feet     Between 300 V and 50 KV - minimum distance required - 10 feet          a) Scaffold on Elm St.: Workers engaged in stucco activities from a 4 frame high tubular welded scaffold approximately 5 feet from a 13KV Primary were exposed to an electrocution hazard, on or about 4/3/2014.    b) Scaffold on Elm St.: Workers engaged in stucco activities from a pump jack scaffold touching the 240V secondary were exposed to an electrocution hazard, on or about 4/3/2014.    c) Scaffold on the roof.: Workers engaged in stucco activities from a 1 frame high tubular welded scaffold approximately 1 foot from the 240V secondary were exposed to an electrocution hazard, on or about 4/3/2014.
