At 515 S. 4th st, Osage City, KS, employee(s) operate a grantry crane equipped with an unserviceable hook in that the safety clip had a broken spring leaving the clip in the open position, exposing employees to struck by hazards from 7,000 lbs molds lifted by the hoist.

At 515 S. 4th St, Osage City, KS, on the south side of the Hi-Bay area, an electrical service disconnect box for the emergency shelter had an approximately 1 inch opening on the upper right side, exposing employees to an electrical shock hazard.
