29 CFR 1926.404(b)(1)(ii):     On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrutpers for personal protection:    a) 504 Harvard Street, east side of roof, on or about 5-10-12:  Employees were operating a hand held circular saw and various other power tools, powered through a duplex receptacle and extension cord that was not protected by a ground fault circuit interrupter (GFCI).

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    a) 504 Harvard Street, west side of roof, on or about 5-10-12:  Employees were working on a 4:12 pitch roof, without fall protection, exposing the employees to a fall hazard of 9 feet 6 inches.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) 504 Harvard Street, east side of roof, on or about 5-10-12:  Employees were accessing a roof using a portable ladder that was not extended 3 feet above the landing surface, exposing the employee to a fall hazard of 10 feet 6 inches.
