On or about November 1, 2012, at the above addressed jobsite, safety inspections were not conducted by persons capable of identifying and correcting hazards so as to assure compliance with the company's safety program.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a. On or about November 1, 2012, at the above addressed jobsite, employees working from the residential roof, which varied in pitch from 8/12 to 12/12, were exposed to falls of approximately 20 feet due to the lack of a fall protection system.
