29 CFR 1910.157(e)(2): Portable extinguishers or hose used in lieu thereof under paragraph (d)(3) of this section shall be visually inspected monthly. Heritage Grain Cooperative had portable fire extinguishers within their facility for employee use in the event of a fire. The company did not perform monthly visual inspections of the portable fire extinguishers to ensure that they were fully charged, undamaged, and ready for immediate employee use.

29 CFR 1910.157(g)(2): The employer shall provide the education required in paragraph (g)(1) of this section upon initial employment and at least annually thereafter. Heritage Grain Cooperative had portable fire extinguishers within their facility for employee use in the event of a fire. The company did not provide an educational program to employees, in order to familiarize employees with the general principles of fire extinguisher use and the hazards associated with incipient stage firefighting.
