29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(IDENTIFY MACHINERY OR EQUIPMENT)(THIS INCLUDES ENFORCEMENT OF PROCEDURES) (a) Tuckey Metal Fabricators, Inc., 150 Stover Drive, Carlisle, Pennsylvania - Employees performed maintenance and servicing activities on various pieces of machinery and equipment, including, but not limited to, the Piranha break press, the Standard shear, the high definition plasma cutter, etc. The employer had a general energy control procedure which indicated where and how to lockout/tagout electrical energy; however, this procedure did not give detailed information on how to control energy sources such as gravitational energy, gases, etc., nor did it give detailed information on servicing activities such as die setting which requires the use of lockout/tagout during some phases of servicing and requires energization with protection by alternative methods during other phases of servicing, on or about May 14, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
