29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    (a)  Cocol Construction, LLC, 90 S. 18th Ave., Brighton, CO 80601:  On and before May 15, 2012, the exposing employer did not ensure that employees working off an 18" aluminum scaffold plank were protected from falling by an adequate scaffold system, or fall protective system.  Employer had set up the scaffold plank between two 10 foot ladders which were positioned on two levels of different heights without any fall protective measures in place such as guard rails.  Condition exposed employees to fall hazards of approximately 15 feet.

29 CFR 1926.451(g)(4)(i):  Guardrail systems were not installed along all open sides and ends of platforms on a scaffold system.     (a)  Cocal Construction, LLC, 90 S. 18th Ave., Brighton, CO 80601:  On and before May 15, 2012, the exposing employer did not install guard rails along all of the open sides of a scaffold system.  Employer had set up the plank between two 10 foot ladders which were positioned on two levels of different heights without any fall protective measures in place such as guard rails.  Condition exposed employees to fall hazards of approximately 15 feet.

29 CFR 1926.451(h)(1):  Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.     (a)  Cocol Construction, LLC, 90 S. 18th Ave., Brighton, CO 80601:  On and before May 15, 2012, the exposing employer did not install toe boards or some other means of protection for employees below the working surface. Condition exposed employees to struck by hazards from falling objects.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder.     (a)  Cocol Construction, 90 South 18th Ave., Brighton, CO 80601: On and before May 15, 2012, the exposing employer did not ensure that a ladder in use to access the second floor of a construction project extened at least 3 feet above the landing surface.  Condition exposed employees to fall hazards of approximately 15 feet while climbing the ladder.

29 CFR 1926.454(a):  The employer failed to ensure that each employee who performs work while on a scaffold was trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.     (a) Cocol Construction, LLC, 90 S. 18th Ave., Brighton, CO 80601:  On and before May 15, 2012, the exposing employer did not ensure that each employee who performs work while on a scaffold was trained by a qualified person in the subject matter to recognize the hazards, specifically fall hazards, associated with the type of scafold being used.   Condition exposed employees to fall hazards of approximately 15 feet while using the scaffold.

(a) Cocol Construction, LLC, 90 S. 18th Ave., Brighton, CO 80601:  On and before May 15, 2012, the exposing employer failed to provide training to his employees utilzing ladders.  The employer failed to provide a training program which  enabled each employee to recognize hazards related to ladders and stairways, and train each employee in the procedures to be followed to minimize these hazards.  Condition exposed employees utilzing ladders to fall hazards of approximately 15 feet.
