29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided: (a) Cocol Construction at 10025 E. Girard Ave., Denver, CO 80231: On and before March 30, 2015 the exposing employer did not ensure that their employees were protected from injury, in that employees were exposed to falls of approximately 16 feet without the use of an adequate fall protection system while performing residential construction work.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Cocol Construction, LLC at 10025 E. Girard Ave., Denver, CO 80231: On and before March 30, 2015 the exposing employer did not ensure that the ladders employees were using to access the upper landing surfaces extended 3 feet above the upper landing areas. This condition exposed employees to a fall hazard of approximately 16 feet.
