29 CFR 1926.1408(a)(1): The employer did not identify the work zone where crane/derrick operations would be conducted:    (a) Jobsite, Veterans Memorial Bridge, Bridgewater, PA:  On or about 05/16/12, the employer did not identify the work zone, with respect to the overhead power lines, where the Manitowoc 222 crane operations were conducted.  The work zone was not demarcated by using flags, range limit devices or range control warning devices nor was the area 360 degrees around the crane, up to the crane's maximum working radius, identified.  An electrical contact (as defined by the 1926.1401) between the crane and 23kv overhead power lines occurred causing electrical shock and burn injuries to a contractor's employee working with the crane operator to rig a piece of equipment.

The height of the overhead power lines was not known to the operator nor was the work zone determined.  An electrical contact (as defined by 1926.1401) with the 23 kV overhead power lines occurred and a contractor's employee who was working with the operator of the 222 Manitowoc crane received electrical shocks and burns when reaching for rigging attached to the crane.
