29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:    a.  On or about May 17, 2012, at the above addressed jobsite, employees were exposed to falls while performing demolition operations from a tubular welded frame scaffold, twelve (12) feet six (6) inches above the ground, that did not have the working level fully planked    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

On or about May 17, 2012, at the above addressed jobsite, employees were performing demolition work from a tubular welded frame scaffold, twelve (12) feet six (6) inches above the ground, that was not provided with base plates or mud sills, thereby exposing employees to falls.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.452(c)(3): Frames and panels shall be joined together vertically by coupling or stacking pins or equivalent means:    a. On or about May 17, 2012, at the above addressed jobsite, the frames of the tubular welded frame scaffold were not joined with stacking pins, thereby exposing the employees to falls.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. On or about May 17, 2012, at the above addressed jobsite, employees performing demolition operations from the building were exposed to falls, of twelve (12) feet six (6) inches, created by the presence of unprotected sides.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part:    a.  On or about May 17, 2012, at the above addressed jobsite, employees performing demolition operations from a tubular welded frame scaffold, twelve (12) feet six (6) inches above the ground, were exposed to injuries from falls due to the lack of guardrails.    BULMARO VEGA DBA VEGA CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.451(g)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311942932, CITATION NUMBER 1, ITEM NUMBER 4A AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 11, 2008, WITH RESPECT TO A WORKPLACE LOCATED AT 4363 N. LINCOLN AVENUE, CHICAGO, IL 60618.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
