29 CFR 1910.147(c)(5)(ii)(C)(1): Lockout devices were not substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools:    On or about June 16, 2012, employees were exposed to amputation hazards when the locks for both areas in the Wendy's department had been keyed for the same key.  Once an area was cleared to remove the locks from the equipment, the key was then available for use in the second area whether or not the work was complete and the area cleared to remove the locks.  This situation was also possible in the frozen areas.
