Strait & Lamp Lumber Company, Inc. - West Locust Street, Newark, Ohio jobsite: On May 18, 2012, an employee delivering lumber to this construction site did not wear, and had not been provide, a "high" visibility vest when working in and near the path of travel on Locust Street, thereby being exposed to a struck-by hazard from vehicular traffic.

Strait & Lamp Lumber Company, Inc. - West Locust Street, Newark, Ohio jobsite: On May 18, 2012, an employee delivering lumber to this construction site was working in and near the path of travel on Locust Street without the posting of traffic signs and markers to warn approaching road-users of construction zones and points-of-hazard.
