29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:    (a)	Lazaros Roofing at 824 Westbourn Ct., Fort Collins, CO 80525:  On and before May 17, 2012, the employer did not ensure the employees were protected from injury, in that employees engaged in construction activities on a roof with a fall hazard of approximately 9 feet 10 inches without the use of a fall protection system.    Lazaros Roofing was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 107886, Citation Number 1, Item 1 issued on February 15, 2012 and became Final Order on February 29, 2012 with respect to a workplace located at 3204 Tabernash Dr., Loveland, CO 80534.    Lazaros Roofing was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number  313412488, Citation Number 1, Item 1 issued on September 3, 2009, and became Final Order on  October 23, 2009, with respect to a workplace located at 2121 Redhead Ct., Johnstown, CO 80534.
