29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems. (a) MAC Construction, Inc. at 2290 S Clayton St, Denver, CO 80201: On May 16, 2012 the controlling employer did not ensure that subcontractor employees were protected from injury, in that the workers removing shingles and installing tarps on the 9-in-12 pitch roof of the church were not utilizing a fall protection system at all times. Workers utilized a fall harness, lanyard, rope grab, lifeline, and roof anchor system. However, when transitioning between roof anchors, workers would disconnect their lifeline from one roof anchor and walk approximately 8 feet before connecting their lifeline to the next anchor. The roof anchors were located near the peak of the roof. This condition exposed subcontractor employees to fall hazards of approximately 19 feet.
