OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees    a. Baxterville, MS -  On or about May 16, 2012 bucket truck # 364 and 470 had mechanical problems that could affect the safe operation of the vehicles.    1. Bucket Truck # 364: The aerial lift had a major hydraulic fluid leak at the base of the lift. The platform/truck bed below the aerial lift base was covered with hydraulic fluid which created a slip hazard for employees walking on the platform/truck bed.    2. Bucket Truck # 470: The horn and windshield wipers were not working; all aerial lift controls were not labeled as to identify their function; insulation markings were not on the aerial lift boom that would have identified the boom as being insulated or non-insulated.
