29 CFR 1910.26(c)(1): good safe work practices in the use of the ladder was not employed by the users to ensure maximum serviceability and safety: a. 818 Curie Drive, Alpharetta, GA (Shop Area) - Employees were exposed to fall hazard. Employees were not utilizing safe work practices when accessing the mobile ladders. Employees were tying back the safety latch using plastic clamps so employees can move the metal ladder while standing on the platform to the next storage rack, on or about 05/23/2012. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: a. 818 Curie Drive, Alpharetta, GA (Shipping/Receiving Area) - Employees were exposed to entrapment hazard. The designated marked and identified emergency exit door located in the rear of the shipping and receiving area was blocked with large heavy equipment, cardboard boxes, stationary workstation and a metal roller conveyor. The items were stored directly in front and on the sides of the designated emergency door. If in the event of an emergency employees would not be able to access the emergency exit door without jumping over the blocked items, on or about 5/23/2012. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

The electrical cord housed on the 250V motor (test equipment) which is used for testing the final product (motors) had damage to the outer jacket insulation, exposing live energized bare conductors, on or about 05/23/12.
