29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part. On or about May 16, 2012, at the worksite this employer failed to initiate and maintain programs necessary to comply with this part (CFR 1926) and to prevent accidents for workers exposed to hazards of falls from improper use of ladders, being struck-by the chips from circular saws and chain saws, not wearing chaps while using chain saws and using a wood chipper that was missing a barrier guard.
