29 CFR 1910.132(a):     Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    a) Employees who purge and do color changes on the plastic injection molding machines were not required to wear coverings for their foreamrs to protect them from burns.    b) On or about May 25, 2012 and employee using a torch to burn plastic off of plastic injection molding nozzels where fire was present was not wearing a flame retardant jacket to prevent burns to the arms and upper body.
