29 CFR 1910.303(f)(2):     Services, feeders, and branch circuits. Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident.    a) Receiving Department: The employer exposed employees to electrical shock hazards of 220 volts as a circuit breaker panel and an electrical disconnect which control the compactor machine were not marked to show what they controlled.      b) Processing/Storage Department: The employer exposed employees to electrical shock hazards of 220 volts as a electrical disconnect which control the shear machine was not marked with voltage ratings or to show what it controlled.

29 CFR 1910.1200(e)(1):     Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:    Written hazard communication program: Employees were exposed to chemical hazards due to the company not developing, implementing or maintaining a written Hazardous Communication Program.  The employees were using the following chemicals such as but not limited to propane, degreaser, bleach, and multi-purpose cleaner.

29 CFR 1910.178(l)(6):     Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.    Documentation & Training: The employer allowed an employee to operate forklifts without certifying that the operator had been properly trained, evaluated, and certified, exposing employees to struck-by hazards.
