29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):    1) Receiving Area - The employer did not certify or evaluate an employee operating the Toyota Forklift truck #3464 model number 7FGCU25 and serial number 92133 and Toyota forklift truck model number 8FGCSU20 and serial number 11309.  Employee was exposed to a struck-by hazard.    The Davis Metals, LLC dba Davis Recycling was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.178(l)(6)), which was contained in OSHA inspection number 443654, citation number 2, item number 1 and was affirmed as a final order on September 13, 2012, with respect to a workplace located at 677 Whitehall Street, SW, Atlanta, Georgia 30310.

29 CFR 1910.1200(h)(1): Employees were not provided information and training on hazardous chemicals or materials in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area:  1) Receiving Area - The employer did not provide training to the employees on the hazardous chemicals used on a daily basis, including but not limited to Propane.
