29 CFR 1926.502(b)(2)(i):  Midrails were not installed at a height midway between the top edge of the guardrail system and the walking/working level:    (a)  Hamon Contractors, Inc. at I-25 & 20th St, Denver, CO 80202:  On and before May 23, 2012 the controlling employer did not ensure that a midrail was installed midway between the top cable of the engineered horizontal lifeline used onsite and the walking/working surface. The horizontal lifeline system is located approximately 6 feet from the unprotected edge and serves as a top rail for workers accessing the west side of its location. Workers accessing the west side of the horizontal lifeline system are exposed to a potential fall hazard due to a missing midrail.
