29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. (a) On or about May 23, 1012, on the south side of the house, employees who were installing new roofing shingles were not using any type of fall protection. The employees were exposed to falls of approximately 8 feet to the ground below.

(a) On or about May 23, 2012, on the south side of the house, employees were using an aluminum extension ladder with side rails that did not extend at least 3 feet above the edge of the roof. The employees were exposed to fall hazards of approximately 8 feet to the concrete below.
