29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpectedenergizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: As of 04/25/2012 the employer failed to establish an effective program for the control of hazardous energy at the facility. Employees perform service and maintenance on equipment such as, but not limited to, the Accu-Kut: plasma laser cutter, the Piranha 88 ton, hydraulic press brake and shear, the Piranha iron worker and several automatic lathes.

The Accu-Kut (hydef) plasma laser cutting machinery, which exposed employees to arcing and flying sparks.
