29 CFR 1910.213(h)(1):     The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock being cut:    Straight Stair Department:  The adjustable guards for the lower portion of the blade on the Rockwell radial arm saw and the Wilson radial arm saw were not maintained so that they automatically adjusted to the thickness of the stock.  The guards were fixed in a position so that they would not touch the stock.

29 CFR 1910.303(b)(2):     Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    1.  Location 300 - Commercial Manufacturing:  Two boxes with prepunched knockouts were used as hanging 220V outlet boxes for the arc welding machines.      2.  Location 700 - Residential Steel/Fiberglass Doors:  A relocatable power tap (RPT) was being used near the Grizzly band-saw behind the Magnum door machine.  The RPT was suspended from one of the power cords that was plugged into it because it was not positioned properly.  Also, wood dust was collecting in the socket openings.

Location 1100 - Exterior/Interior Wood Doors:  A 30-foot retractable cord reel was used to power the Lynn drill press.

29 CFR 1910.178(a)(4):     Modifications and additions which affected capacity and safe operation were performed by the customer or user without manufacturers written approval, and the capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly:    Location 500 - Receiving/Shipping Warehouse:  6-foot fork extensions were being used on the forks of the Nissan 50 Optima LP forklift to move oversized loads.   No written approval from the forklift manufacturer was obtained by the employer.  The addition of fork extensions to carry long loads can increase the distance of the center the load from the forklift which decreases the capacity of the forklift.
