Broadway & Mullan Construction Site: On or about May 24, 2012, and at times prior thereto, employees were exposed to the hazard of becoming struck-by oncoming vehicular traffic while replacing concrete in the street work zone and were not provided with an advanced warning system. (b) W. Broadway & Mullan Construction Site: On or about May 24, 2012, and at times prior thereto, employees were exposed to the hazard of becoming struck-by oncoming vehicular traffic while replacing concrete in the street work zone and were not provided with high-visibility clothing.
