29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. a) Jobsite: 660 E Erie Street, Philadelphia, PA. - On or about 4/5/12, employees exposed to fall hazards were not instructed to avoid such unsafe conditions. Employees were conducting roofing operations near the edge of a roof of a two story commercial building, approximately 30 ft. above ground level, without any fall protection.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Jobsite: 660 E Erie Street, Philadelphia, PA. - On or about 4/5/12, no fall protection system was used to prevent the employees from falling off the side of the roof. Employees were performing roofing operations within 1 ft. from the unprotected sides, and approximately 30 ft. above ground level.
